The Medical Leave request process and all related forms are available through eHealthSafe.
For Active & Inactive Employees.
NOTE: if you haven't logged into the UP Employees' site for 30 days or more, you may need to contact OSS at 402-544-5555 or toll-free at 800-621-8953 to reactivate your user ID.
Employees have two options to submit documents to Health & Medical:
1. Upload to eHealthSafe (best option)
2. Fax using the barcoded coversheet
Documents must be in a PDF or TIF format, Pictures of documents will not work unless you convert them to PDF prior to uploading or e-mailing.
Do not Upload or e-mail documents as separate pages. Upload or e-mail all documents as one attachment.
Employees may use available accrued vacation while on a leave.

Submit a TRM Ticket for specific questions regarding Medical Leave or a Medical Leave request that can't be resolved using the Self-Service Information provided above. If you are unable to submit a TRM ticket, please contact the Health and Medical Services at 402-544-7011 Option 1.
Medical Leave related questions are directed to the Leave Management Team and are responded to in the order they are received.
